Informing Adults With Back Pain About Placebo Effects: Randomized Controlled Evaluation of a New Website With Potential to Improve Informed Consent in Clinical Research

An a priori power calculation was conducted using G*Power (Heinrich Heine Universitat Dusseldorf) [37]. Assuming an effect size f=0.15 (based on unpublished pilot data), power 0.8, and alpha .05 for a factorial analysis of variance (ANOVA) required a total of 351 participants, and assuming 5% dropout required 369 patients to be randomized. Overall, 2 websites about placebo effects were developed for this study: a person-based website and a control website.

The Homework Intervention Strategy (e HIS) program, an online adaptation of the Kinsey Institute Homework Intervention Strategy (KIHIS) [22], combines a focus on pleasure and fit-and-feel in condom promotion with the benefits of an online intervention. In the KIHIS, during the session with the instructor, men are given the correct condom use instruction and are asked to practice condom application on a penile model.

This was a Web-based questionnaire study (that used between and within subject analyses). The study employed a quiz to collect data. A draft quiz was initially trialed on 6 pilot study participants and was then refined on the basis of their feedback during individual “think aloud” sessions. “Think aloud” is a commonly used protocol for usability testing of an intervention [36].

Informing Patients About Placebo Effects: Using Evidence, Theory, and Qualitative Methods to Develop a New Website

Our intention was to develop a resource that could potentially be used across a large number of clinical trials and/or adapted for use in specific trials. However, because the effects and mechanisms of placebos differ across symptoms and diseases [24], we chose one clinical target to focus on for this version of the website. We chose placebo analgesia as pain is relevant to a large number of clinical conditions and placebo analgesia is a well-documented placebo effect [4,25-27].
